Telehealth Instructions

Beginning 2/21/23, all new patients will be seen in the office.  Follow up appointments may be done either virtually or in-person.  Please visit the Telemedicine Update for more information.

Insurance and Payment


Dr. Putnam does not participate with any health insurance networks,  including Medicare and Medicaid.  He is considered out-of-network for all plans.  Patients are responsible for all fees associated with treatment, and are due on the day of service.  

We do not submit insurance claims for out-of-network benefits, but we can provide a “super bill” upon request which will have all of the information you need to submit to your insurance company for possible reimbursement for out-of-network services.


To pay your bill, please use HealowPay.  You can use the statement code sent to you on your bill, or you can use your Personal Information to see your balance and pay.


We require that a credit card’s information be kept on file (stored electronically with

secure, encrypted, HIPAA-compliant software) to be used for charges incurred for late

cancelations, missed appointments, returned checks, or past due account balances,

without additional authorization.

Location and Directions


Dr. Putnam is located in Chevy Chase MD, in the Chevy Chase Building located at 5530 Wisconsin Avenue.  The office is on the 12th floor in Suite 1255.  Please be mindful that there is traffic at all times of the day, so leave enough time to get to your appointment on time.


There is parking (for a fee) at the Chevy Chase Building.  Please leave extra time for parking as the lot is often crowded and slow.  Additional parking is located across the street at Saks Fifth Avenue.


His office is located 3/10 of a mile from the Friendship Heights Metro stop on the Red Line.  When exiting the Metro, follow the signs for Western Ave, and then walk North on Wisconsin Ave.
