Letter Request Form

If you need a letter from Dr. Putnam, please send him a message through the patient portal.  Please include the following information:

1) Your email address

2) How we should send the letter… to your email, to another email, by mail.

3) Purpose of the letter- Please let us know why you need a letter, what needs to be in the letter, how much detail you want us to provide, and any other information that might be helpful.

4) If you have any forms you need to send to us, you can email them to office@putnam-psychiatry.com.

Because of the time required to write letters, a standard charge of $35 will be billed.  If a letter is more lengthy or complete, the fee may be greater, but you will be informed of this prior to being billed.
